ADDITIONAL INFORMATION

There is wheelchair access to the Health Centre via the lift in Main Reception of Founder's or, by prior arrangement, via the North Quad side entrance to the Health Centre.

If your disability/illness does not enable you to attend the Health Centre or to collect your drugs from the local pharmacy please inform one of the nursing sisters so that alternative arrangements can be discussed.

Home / room visits are occasionally necessary. Contact with the duty nurse who will assess the severity of the illness. She may decide to visit herself, arrange for transport to bring the patient to the Health Centre or discuss with the duty doctor if appropriate.

CHANGE OF ADDRESS

Please inform us if you change your address or telephone number. If you move outside our Practice area we will be able to supply information regarding your nearest Doctor's Practice.

If you need to be contacted during vacation time e.g. for a hospital appointment or investigation results, please leave a forwarding address/telephone number.

COMMENTS AND COMPLAINTS

Comments on the service are welcome. Should you have any comments, complaints or questions about the service given to you please do not hesitate to speak to the Nurse or Doctor concerned. You may prefer to discuss any grievance with the Medical Officer, Senior Nurse or the Practice Administrator who will be able to advise how to proceed should you feel that your complaint has not been addressed satisfactorily. You may also contact the Practice Manager, Mrs Jane Thompson, at the main surgery on 01753 865773.

ADVICE FOR NEW STUDENTS

During Induction Week you are invited to register with the General Practitioners at the Health Centre, if you are living in the catchment area i.e. all Halls of Residence, Englefield Green, Old Windsor, Windsor and parts of Egham. The receptionist can provide information for students residing outside this boundary.

Registration Hours: Monday 22/9/08- Friday 26/9/08 9.00am-12.30pm and 2.00pm-4.00pm

TO REGISTER
Please bring:

  • Your completed questionnaire found in the centre of your University Registration Pack.
  • Your National Health Service (NHS) Card or NHS number.
  • If you are from overseas you will require your passport and a Visa which is valid for at least 6 months.
  • An up-to-date passport photograph.
  • You will be given the opportunity to discuss matters concerning your health with a member of the nursing staff. There is a delay following registration before your NHS records are received. Your questionnaire is the only information we have when you first arrive. If you are taking regular medication (including oral contraceptives) you should bring sufficient supplies with you to last for at least a month. If you are from overseas please bring with you full details of all medicines so we can prescribe an equivalent preparation (not all medications will be available in the UK).
  • You should also bring a small personal first aid kit including plasters and your preferred painkillers.
  • As there is no University dentist you are advised to have a routine dental check before leaving home. In an emergency the Health Centre can advise on local dentists.
  • ALL INFORMATION ON YOUR QUESTIONNAIRE IS STRICTLY CONFIDENTIAL AND REVEALED TO NO-ONE WITHOUT YOUR PERMISSION.
